Job Description:

A Care Coordinator is responsible for providing Prenatal Care Coordination (PNCC) services and ongoing Child Care Coordination (CCC) services to clients as mandated by the Wisconsin department of Health and Human Services. The Care Coordinator will work with mothers and the recipient’s family to promote positive parenting and assist in improving low birth outcomes among women who are deemed at high risk for poor birth outcomes and prevent child abuse and neglect as well as supervise the coordination of these services.

Job Responsibilities:

• Recruit, enroll, and maintain a caseload of 10 families minimum

• Conduct home visits with all enrolled families to improve healthy pregnancy outcomes, parenting skills, and to help them provide learning skills that enhance their child’s growth and development

• Conduct family assessments with each parent to determine strengths and weaknesses

• Provide maternal parenting and child development information relevant to their child’s growth and development level

• Document all contacts and information to ensure each family’s file is current

• Act as an advocate for parents and children in the community

• Attend orientation, training, development, and administrative meetings and/or required functions held by the agency

• Adhere to all Bellies & Babees document policy and timelines to make sure paperwork is up to state standards
